Liability Waiver

I understand and acknowledge that the activities I may voluntarily engage in for MINIs On Top LLC (MOT) bear certain known risks and unanticipated risks which could result in injury, death, illness or disease, physical or mental damage to myself, to my property or to spectators or other third parties. I understand that any form of recreational activity can result in loss or damage to personal property, injury or fatality due to any number of seen or unforeseen events.

Being aware that these activities entail known and unknown risks of injury to myself and a risk of injury to spectators or to other third persons, I expressly agree, covenant and promise to accept and assume all responsibility and risk for injury, death, illness or disease, or damage to myself or to others, and to my properly arising from my participation in this activity and my use of these facilities. My participation in these activities is purely voluntary, no one is forcing me to participate, and I elect to participate in spite of the known and inherent risks.

I hereby voluntarily RELEASE, WAIVE, DISCHARGE, AND COVENANT NOT TO SUE, MINIs on Top LLC (MOT), and their respective officers, servants, agents, and employees from any and all liability, claims, demands, actions and causes of action whatsoever arising out of or related to any loss, damage, or injury, including death, that may be sustained by me, or to any property belonging to me, while participating in physical activity, or while on or upon their premises.
